As to national flags, the former (blue ensign) national; flag of the Bahamas had a ship flying the St George's cross (I think). Don't think the current "ensign" style national flags of Bermuda or St Helena have flags on them, though, even though their badges feature ships.

The old Union of South Africa flag, with its three small flags in the centre stripe, is an appropriate example of what is really meant by the term "flag on a flag"? And that a flag in the canton, or truck corner, while in fact a "fof," is a rather special case -- simply called a canton, I suppose -- in that it is always in the same location, and, like the many UK-ensign colonial (or ex-colonial) designs, French colonies like Wallis et Futuna, the county flags of Liberia, and countless presidential, royal and ministry flags, usually signifies a subsidiary relationship.

Flags like that of Haiti, Ecuador, and the greatest example of them all, the Organization of American States, show as part of their symbolism pictures of actual flags on poles.

There is also a sort of intermediate form, a flag shown waving or fluttering in a breeze (flag books, flag sheets and almanac pages often use this form). These are usually shown clearly enough to be easily recognized, but not exactly as flat and square as, say, an actual flag hung on a wall.
